I currently collect and process the following information:
For initial enquiries about counselling with Grow Your Potential Psychotherapy, I currently collect and process the following information:
- full name
- email address
- home address
- telephone number
How I get this information and why I need it
- The personal information is given to Grow Your Potential Psychotherapy by you, to allow me to contact you to arrange counselling. This information may be shared, only with your consent, if it is appropriate to refer you on to another professional.
How I get your personal information and why I collect it
Most of the personal information we process is provided to us directly by you for one of the following reasons:
- When you first make contact about counselling, the information I collect allows me respond to you, for example your full name, email address and phone number, for the purpose of arranging counselling. This personal information may be stored on my WordPress database and in a contact record in Gmail my email provider.
- I will only keep this personal information for as long as necessary, so if after a month there has been no further communication between us, I will delete it. If you decide not to go ahead with counselling, and you would like me to delete this information sooner, just let me know.
- UK General Data Protection Regulation (UK GDPR) says I must have a lawful basis for processing your personal data. My lawful basis for holding and using your personal information is legitimate interest, which means that you give me your personal information via the internet or over the phone, in the expectation that it will be stored electronically in order to arrange counselling.
How it is stored
- All my devices are password protected using LastPass password manager which creates and stores secure passwords and I use two factor authentication where possible. My data is stored using Microsoft Office 365 which is protected by state-of-the-art encryption. My email is handled by Gmail which provides a secure service. Any pen and paper notes that I take are shredded, and I keep electronic data secure by logging off my computer when I am away from my desk.
Your data protection rights
Under data protection law, you have rights including:
Your right of access – You have the right to ask us for copies of your personal information.
Your right to rectification – You have the right to ask us to rectify personal information you think is inaccurate. You also have the right to ask us to complete information you think is incomplete.
Your right to erasure – You have the right to ask us to erase your personal information in certain circumstances.
Your right to restriction of processing – You have the right to ask us to restrict the processing of your personal information in certain circumstances.
Your right to object to processing – You have the the right to object to the processing of your personal information in certain circumstances.
Your right to data portability – You have the right to ask that we transfer the personal information you gave us to another organisation, or to you, in certain circumstances.
You are not required to pay any charge for exercising your rights. If you make a request, we have one month to respond to you.
